Physician Referrals
Boomerang Health is pleased to offer a variety of services, both rehabilitation and physician. Our rehabilitation services do not require a doctor’s referral, but our physician services do. If you are a doctor, please complete the “General Referral Form to Boomerang Health”, which can be found below, and either email it to us (can do this directly on the form itself) or fax it to our office at (905) 553-8120. We will then be in contact with the patient, schedule an appointment, and notify you of the booking. Please also forward all notes, reports (labs/imaging), current growth chart, and any documentation related to the referral as soon as possible.
